What Is a Bunion (Hallux Valgus)?
A bunion is a progressive deformity of the first metatarsophalangeal (MTP) joint — the joint at the base of the big toe. It develops when the first metatarsal bone drifts inward (toward the midline of the body) while the big toe angles outward (toward the smaller toes). This creates the characteristic bony prominence on the inside of the foot.
The term "bunion" typically refers to the inflamed, swollen bump (bursa) that forms over the prominent metatarsal head as it rubs against shoes. But the underlying problem is a complex deformity involving changes in bone alignment, joint capsule, ligaments, tendons, and the small muscles of the foot.
As the deformity progresses, the muscles that normally stabilize the big toe joint actually begin to worsen the problem — pulling the big toe further out of alignment. This is why bunions tend to get worse over time without intervention.
What Causes Bunions?

- Balance problems and increased fall risk — the big toe plays a critical role in balance and propulsion during walking. As the deformity worsens, balance is compromised
- Pain that worsens with prolonged standing or walking — similar to the activity-related pain pattern seen in plantar fasciitis and Achilles tendinopathy
How Are Bunions Diagnosed?
Bunions are diagnosed through a combination of physical examination and imaging.
Physical examination:
- Visual assessment of the deformity — the degree of big toe deviation and prominence of the metatarsal head
- Assessment of joint range of motion — checking for stiffness or crepitus (grinding) that may indicate joint degeneration

Types of Bunion Surgery
